Sub-contractors


New Providers

Penn Foundation’s EAP relies on sub-contractors throughout Pennsylvania (and many other states) to help provide counseling services to our members.  If you are interested in participating in our provider network, please complete an Individual Provider Application.  We will inform you if your application has been approved.  If it is approved, you will be sent a provider contract.  Once this is signed, we can begin to send you referrals.  Please contact us with any questions/concerns.
